You’ve made the team. This form replaces the paper packet at Signing Day — fill it out at home so we can spend Signing Day getting kids fitted for uniforms instead of pushing paper.

I acknowledge that soccer carries inherent risk of injury and release ACE Soccer Club, its coaches, volunteers, and affiliates from liability for injuries sustained during participation.

I have read and understand the concussion fact sheet and agree to follow ACE Soccer Club's concussion protocols.
